True History — Emergency Paper Procurement Form

IMPORTANT NOTICE REGARDING THIS FORM

We are aware of the following situation:

  (a) To order the True History DVD, you must complete Form TH-DVD-001 (the DVD Order Form).
  (b) Form TH-DVD-001 must be printed on paper.
  (c) You do not have any paper.
  (d) Therefore, you require paper.
  (e) To order paper, you must complete this form (Form TH-PAPER-001).
  (f) This form must also be printed on paper.

We acknowledge that step (f) presents a logical challenge. Our recommendation is to borrow a single sheet of paper from a neighbour, colleague, or passing stranger in order to print this form. Once your paper order has been fulfilled, you will have sufficient paper to print the DVD order form as well.

We apologise for any inconvenience caused by the material requirements of the physical world.

Section 2 — Paper Requirements

Please indicate the paper you require. We supply standard A4 80gsm white printer paper in the quantities below. One sheet is sufficient to print Form TH-DVD-001. We recommend ordering at least five sheets in case of printer errors.
